This is high time when applicants are making rank order list. I hope this post may help them making their decision. This is based upon our observation, discussion with residents and fellows in different programs in last 5 years. you can deviate based on individual preferences and circumstance):
Following is the suggested order of rank list in descending order based on fellowship, type of program and visa perspective:
1. University program with fellowships that are given to their own residents and with H1 (e.g Kalamazoo University, Nebraska University ) is most preferred and shoul dbe at top.
2. University program with fellowships that are given to their own residents and with J1 (e.g Oklahoma University) is next in list.
3. Community program that is H1 but has in house fellowships given to its own people . (e.g Montefiore Wakefield ( for GI) Guthrie Robert Packer ( for cardio) , Geisinger Medical Center, Danville. For cardio ) is third on ranking.
4. University program with J1 and fellowships that are not given to their own residents (e.g Toledo University as per this match stats) is next on list.
Community programs with H1 with no fellowships BUT better city like New York /Chicago (Abington hospital) is BETTER THAN
5. Community program with H1 with no fellowship in small town (e.g Marshfield clinic) is next on list.
Community program with J1 (e.g Brookdale hospital) is last on list.
It is time to rank lists of programs. This should be done with great wisdom. Please consider following while ranking programs:
1. Always rank them way you want; do not think that how was interview; if you want to be at a program and Program is John Hopkins and you had a terrible interview; still rank it higher no matter what are your chances.
2. You can rank 2 specialties also but rank most favorite specialty always higher. This is wrong concept that university program of peds is better than community of medicine if you want to go to in medicine. In this case , even community program of medicine should be higher than university program of peds.
3: i will mention one formula here:
"a community program with h1 VISA and HAVING A FELLOWSHIP IN HOUSE THAT YOU ARE INTERESTED IN and the program takes in house residents; is better than a J1 university program that has fellowships but do not take their own residents"
This is specially true for big 4 fellowships of USA. that is
gastroenterology, pulmonary/Critical care; cardiology; hematology/oncology.
Example is getting cardiology at Guthrie Robert packer will be easier than getting cardiology at Oklahoma University as Oklahoma might prefer more competitive candidates from outside.
4. Following factors should be considered. while finalizing rank list. They are in order of the descending order in their significance.
a:Fellowship:
very imp factor; if you want to have a particular fellowship; choose program higher which has that particular fellowship as in-house as it is easier to get in there. Otherwise, rank university higher as they have inside fellowships. On j1 u always have to do waiver-no matter u do fellowship 1st n waiver later or vice versa-difference is that if u do fellowship 1st, u earn more in waiver. On other hand, if u do waiver 1st, u have less chances to get in fellowship after 3 yr of working in a non teaching waiver program in small town. (waivers are usually in small community non teaching hospitals in towns)
On other hand, if you have h1 program and also in-house fellowship in same program, that is ideal combination. u CAN HAVE EITHER DIRECT FELLOWSHIP; OR EVEN IF NOT; U CAN GO FOR hospitalist job in good academic programs that also give GREEN CARD THAT YOU CAN GET IN 2 YEARS AND MEANWHILE do research in a good academic program and apply fellowship later that has very high chance of getting it without any thing like waiver
b-University vs community:
its imp factor and most people go for university which is not bad approach but one factor should be kept in mind-that is visa status that will be explained in my next line. i will mention one formula here:
a community program with h1 VISA and HAVING A FELLOWSHIP IN HOUSE THAT YOU ARE INTERESTED IN and the program takes in house residents; is better than a J1 university program that has fellowships but do not take their own residents.
c. Visa:
very imp factor in ranking; IMGs rank H1 higher than J1 ; Ji are mostly universities and have higher chance of direct fellowship but if not matched then you have to go to waiver which most people do not like. H 1 has benefit that you can go to hospitalist tract and get green card in 12 months and then apply fellowship.

d: Area:
important factor especially females. Prefer those cities where you have spouse or family; Otherwise, some people like cities and others like towns; its all personal preference , Another thing; many people falsely label some ares crime areas; i think before making this impression and deciding based on this hear-say; always ask residents already living in that area. Like, many people say about Bronx that it is unsafe although nothing is wrong there. Many people say same about Baltimore; I would always ask residents of program about this
I would mention my example here. it may help others:
I wanted H1, and GI fellowship. There were few J 1 community programs in list.
I ranked Montefiore Wakefield above Marshfield although Marshfield is bigger program with research. Reason was GI fellowship as Montefiore has in-house 1 seat of GI, and other reason was new york city that is better place than Marshfield. That worked.
Consider all above factors while making decision. You can send your list to seniors and ask their opinion but it all depends on your personal circumstances.
I will again say, your performance in an interview should not be considered while ranking; all that should be considered is your liking for that program.

USMLE Step 1 Experience (262) by Rizwan Zafar

I am writing this experience because I think it might help others avoid the mistakes I made during my preparation. I started preparing for step 1 after my final year result was declared, and I took a long time preparing for it. I did all  subjects from kaplan except pathology, for which i did goljan and it was a big mistake as it took me more than 3 months to complete it along with my medicine rotation and in the end I almost retained none of it. I hadn't completed my first read until the end of my 3rd rotation of house job i.e 9 months for the first read with house job.  I read every subject from kaplan(except patho) along with video lectures for most subjects followed by relevant First aid portion after each subject. For patho I did goljan along with its lectures followed by first aid of patho.            Then I  left my last rotation of house job in view of speeding up the things a bit (no use at all) only to resume it 4 months...
